The following form should be completed by ALL new brothers immediately following their initiation. Chapter Officers should update their roster on the Member Site, in the “Chapter Officers” space and ensure this form is shared with new brothers.
This form requires payment of the New Brother Joining Fee. If your chapter would like to have the new brother joining fee billed to the chapter, you must email us to make this request. (Note: There is a $35 processing fee per registration for having the Joining Fee billed to the chapter.)
