All Brother Must Complete This Form Once Initiated

Chapter officers must ensure ALL brothers complete this form with accurate contact information and Initiation Ceremony date.

This form requires brothers to pay the Initiation Fee. (If your chapter would like to have the Initiation Fee billed to the chapter, email us to make the request. Please note there is a $35 processing fee/brother for billing the Initiation Fee to the chapter.)
