All Brother Must Complete This Form Once Initiated
Once brothers are initiated, chapter officers must ensure ALL brothers complete this form and update their chapter’s roster on the Member Site (in the “Chapter Officers” space) accordingly.
This form requires brothers to pay the Initiation Fee. (If your chapter would like to have the Initiation Fee billed to the chapter, email us to make the request. Please note there is a $35 processing fee/brother for billing the Initiation Fee to the chapter.)
